feat(intelligence): project decision outcome feedback loop - #135

Outcome

Closes the public evidence-to-outcome read path by adding Decision, Outcome, and governed Feedback-proposal families to the unified Intelligence resource plane.

Exactness

• validates PREPARED immutable envelopes before projection
• resolves Decision → Brief provenance to the Brief's public content identity, not Core's storage hash
• projects Outcome → Decision and Feedback → Decision + Outcome exact provenance
• preserves product, subject, as-of, available-at, cursor, and page boundaries
• keeps unknown Decision subject types visible only as explicitly degraded truth
• centralizes all current contributor composition in one supported Core host factory

Verification

• repository-wide Ruff lint and format checks pass
• 79 focused resource-plane, schema, HTTP, boundary, exact-eleven, monitoring, decision/feedback, and projection tests pass

Scope

No Action projection, committed feedback-state projection, World/Market code, connector implementation, new store, authority widening, MCP change, Atrium implementation, merge, or release.
